We gladly accept new patients and welcome everyone. Our front desk scheduling coordinator will schedule your first visit and ask for basic information as well as your insurance information. This will allow us to confirm insurance eligibility and benefits prior to your appointment.

There will be new patient information to fill out as well as a health history. Please know the medications you are currently taking and any allergies you have. New patient forms can be printed out from this page and brought with you to your first visit, or we can mail them to you prior to your appointment. You are always welcome to fill them out here at your first visit, just come in a few minutes early.

We want your first visit to be a true quality experience and to provide you with information about your dental health, we will schedule your first visit with one of our doctors. At this visit you will have a complete examination and evaluation by the doctor as well as update your radiographs. If you have bite wing radiographs (x-rays) from a previous office that were taken within one year and/or a panoramic x-ray or full series, please either have them forwarded to our office prior to your appointment or bring them with you. This will allow us to only update what is necessary. We can also check with your insurance company about radiography frequency if you are unsure.

Prophylaxis (cleaning) appointments will be scheduled with one of our dental hygienists after your initial appointment. Future appointments with the doctor are scheduled based on your needed treatment. At times we will schedule a Review of Findings appointment with the doctor and treatment plan coordinator. This will provide you with treatment options, order of treatment, insurance benefits and financial options.

All of the doctors are available for new patients, please let us know if you have been referred to one of them from a friend, family member, co-worker or word of mouth, we will be sure to schedule you with the doctor of your choice.
